Blood-brain barrier disruption occurs in pathologic states such as the recovery from stroke, infection of the brain, or brain tumors. In addition, there is evidence that the BBB is disrupted in states of severe stress or emotionality. This phenomenon may occur in patients who are acutely admitted to a hospital under severe stress. In this setting of transitory BBB disruption, drugs that are normally excluded from the brain may cross the disrupted BBB. This process may underlie the high incidence of CNS neurotoxicity in acutely hospitalized patients. The biochemical basis of BBB disruption is poorly understood and is an area in need of additional research. Once the biochemical basis of this process is elucidated, new drugs may be developed to prevent BBB disruption in states of severe emotionality.
