Paralysis of Triceps Surae Muscle in Poliomyelitis

Flaccid lower motor neuron paralysis of triceps surae is very disabling. The patient lacks push-off, and walks with a calcaneus limp. The lack of push-off during the third foot rocker leads to an upward displacement of the anterior end of the calcaneum and head of talus. There is increase in the range of dorsiflexion and the normal posterior heel prominence disappears. Compensation by other plantarflexors such as tibialis posterior, long toe flexors, and the peronei result in equinus of the forefoot and eventually a calcaneo-cavus deformity. Contracture of the short toe flexors and plantar fascia increases the cavus deformity.

Problems of the Feet

Underlying neuromuscular condition spinal dysraphism (spina bifida, lipoma, tethered cord, diastematomyelia), Charcot-Marie-Tooth disease, Friedreich's ataxia, or cord tumor. The workup includes family and neurological history and exam, weight-bearing radiographs of the feet, and strong consideration of a referral to the orthopedist. Corrective shoes and inserts are not effective for treating cavus feet. Surgical management, best undertaken after age 4 or 5 years, is directed toward medial and plantar release (plantar fascia, short flexors, adductor hallucis) followed by weekly cast changes to gain full correction.13

Method of Evaluation

In patients with chronic heel pain, magnetic resonance imaging (MRI) regularly shows involvement of the calcaneal insertion of the plantar aponeurosis (Berkowitz et al. 1991, Grasel et al. 1999, Steinborn et al. 1999). The diagnosis of plantar fasciitis is straightforward, even more so when an inferior calca-neal spur has been detected. However, the spur may be an incidental finding (Lapidus

Clinical Evaluation

A high index of suspicion for navicular stress fracture is necessary in the approach to treating an active patient who complains of foot pain. In other words, if a weight-bearing athlete experiences an insidious onset of midfoot pain, a stress fracture must be ruled out, even if radiographs are normal. Torg and colleagues 8 have noted an average period of 7 months between the onset of symptoms and the diagnosis, in their series.

The sports most commonly associated with Sever's disease are soccer and running. The disease presents with unilateral or bilateral (60 ) posterior heel pain in the 8-to 13-year-old athlete. It is associated with accelerated growth, tight heel cords, and other biomechanical abnormalities. Patients present with tenderness at the insertion of the Achilles tendon on the calcaneus. Radiographs may show partial fragmentation and increased density of the os calcis, thereby ruling out other rare causes of heel pain, such as unicameral bone cyst or a stress fracture. Activity modification, stretching of the gastrocnemius-soleus complex, ankle inverters and everters, and heel cups have all proved helpful. Children may return to sports without limitation 2 to 4 weeks after symptoms resolve.

Domastoid muscles are detached from the sternum and clavicles bilaterally (Figure 34-2A). The pectoral muscles are elevated on either side from the midline of sternum to expose the cartilages of the first and second ribs and medial portions of the first and second intercostal muscles (see Figure 34-2A).
